The reablement and care service provides long-term care support to tenants in Extracare schemes and short-term reablement care support to Newport residents living in their own homes to help them to live independently in their own home.
Personal care support (excluding nursing care) is provided to people living in Wellwood, Willowbrook, Glyn Anwen and Capel Court Extracare schemes who meet the council's eligibility criteria, including:
- assisting with personal care, showering and dressing
- assisting with continence
- assisting with preparation of snacks and drinks
- assisting with mobility
- prompting medication in line with Newport’s medication policy
The service provides short-term care support for people to have their care needs assessed, perhaps after a stay in hospital, after a fall or if there is a deterioration in general health and ability.
The support is reviewed each week and if longer term support is assessed as necessary another provider will give deliver this for a charge.
Support can be provided to encourage independence with bathing and dressing, to improve mobility and to assess the person's ability to prepare a meal.
The service is available every day of the year between 7am and 11pm.
