This entry form should be completed and submitted no later than 1st May 2006. Receipt of your submission will be acknowledged, together with a request for the entrance fee of �50, a copy of your birth certificate and the two references.
Please make cheques payable to the Newport Pianoforte Competition. Although cheques are preferred, entrants may also pay their entry fee by bank transfer.
